The members of Parke Davis (India) Limited Superannuation Scheme (the “Scheme”)
If you have reached at your normal retirement age (60 years), you are now eligible to claim the pension under policy no. 1177. Kindly send the below documents to initiate the claim with LIC of India (“LIC”).
Requirements of superannuation claims are as under:
1) Pension claim form
2) GSCA(2) form
3) ECS form
4) Copy of PAN card
5) Copy of Aadhar card
6) Cancelled cheque
Please be informed that the pension is payable only as per the benefit rules of the Scheme, LIC will calculate and pay the annuity if the member is eligible.
We would request you to kindly provide a copy of employment letter/Payslip/VRS letter issued by Parke Davis Limited at the earliest at [email protected] along with above pension forms to update the KYC in the company’s records. Also, kindly send original signed forms with supporting documents at below address –
